• Please bring your government photo ID such as a driver’s license or any state-issued ID to your appointment. 

• Please fill out and bring the NEW PATIENT FORM to your first appointment.

• If available, please bring any recent lab testing in the last 6 months such as CBC, fasting blood sugar, cholesterol and thyroid level.

• Most insurance companies don’t cover our preventive treatment so for simplicity we do not accept insurance. We accept cash, or cards. 

• All payments are made in full on the day of the appointment.
